Needing social security card

I have a participant whose boyfriend was in jail for awhile and his previous girlfriend was given his wallet with all his information in it. She will not return it and she is in Chicago. He is trying to find a job but needs to get his social security card so that he can get an id yet all he has for information is his medical record from jail, print out from jail of his picture, social security number and birthday with his information listed on it. He also has mail. The social security office has turned him down over and over saying he doesn't have the correct information needed. Any suggestions of direction on this would be greatly appreciated.
